How to Complete the Payment Form:
- Enter the payment amount on the Stripe Payment form:
- If you received a paper statement, find the payment amount on your statement and enter it here.
- Or, check the text message you received for your payment amount.
- Click/Tap The “Pay Securely” button: Payments are securely processed by Stripe. Additional information is required to complete the payment.
- Enter your full name: Please provide your first and last name as it appears on your statement.
- Enter your date of birth: This helps us verify your identity.
- Enter your chart number (optional): You can find this number on your paper statement. If you do not know it, you may leave this field blank.
